WHY DOES MY NOZZLE ASSEMBLEY KEEP FALLING OFF

When I run the dishwasher. I get a buffering sound. Like someone is loose. Has happened on several occasions. Not because of loose bowl anything. This sound happens every time I use it. This was not always the case. Took the top and bottom spray wands out. They do not appear to be plugged or anything. Any ideas?
