Inside the oven compartment at the top there is a square hole with what appears to be insulation sticking out. Is there a cover that should be on the opening?

There is usually no cover for this opening.
All ovens are vented one way or another. You would not want to heat up the air in a sealed chamber because of thermal expansion. It would explode due to the heat expanding because it has no way to vent. A vent is also needed to vent fumes and by-products that burn off during the self-cleaning process if applicable. I am an HVAC tech by trade so it wasn't difficult to diagnose problem with safety valve. The hard part was being without an oven for a week. Repair was simple and effective--oven works good as new. Shut off gas and unplugged oven. Removed floor of oven between broiler and bake section, used wrenches to disconnect gas line from safety valve in back of oven, used philipps head screwdriver to remove burner assembly and gas valve, reversed process to replace. Checked out operation--OK.
Turned off power, took back off stove removed old probe. I had to splice wires since new probe ends did not match up. the instructions were to splice the wires. I had the breaker off for about 3 hours. this is not in the instructions, but being a widow, I had to ask a few people why it would not come on after I connected the wires. After waiting, I turned the breaker on and my range came back on.
Repair was quite easy, as long as you have the instructions for removing the door. The screws that hold the bottom of the door to the fixed hinge are placed into the side of the hinge assembly after removal; this holds the door at a 45 degree angle for easy removal. After that, just place the door on a flat surface and dismantle the door from the inside. The inner glass is contained in a metal frame with another piece of glass; this frame comes apart easily using the clips in each side. The door assembles easily after the glass is replaced; just take your time and give yourself enough room to lay out the parts as you go. Good luck!
